Responsibilities of the Registrar

The Office of Vital Statistics and Registry registers vital events and maintains the following vital records that occurred in Milltown, NJ:

  • Marriages
  • Births
  • Deaths
  • Civil Unions
  • Domestic Partnerships

Vital records are also available from the New Jersey State Vital Records Office. The State can also be contacted at 609-586-9316.

For information about vital records from May 1848 – 1917 visit the State, Archives website. The Archives also holds microfilm copies of births from 1901-1923, marriages from 1901-1940 and deaths from 1901-1940. These materials are available for in-person use only.

Please note, New Jersey law protects and restricts the release of vital records. Vital records are not public records and cannot be searched online. 

Marriage Applications

The couple and a witness must all be present to process a marriage application. Please note that blood tests are not required. There is a minimum waiting period of 72 hours between time of application and time of marriage. 

Marriage applications are by appointment only. An appointment can be made by e-mailing Catherine Beardsley or calling 732-828-2100 extension 126.

Request Certified Copies

Certified copies of births, marriages, civil unions and deaths that occurred in Milltown Borough can be requested through the Registrar of Vital Statistics through an application. Certified copies are only available to the following:

  • Child
  • Current Spouse
  • Grandchild
  • the Individual on the Requested Record
  • Individual's Parent(s)
  • Legal Guardian
  • Legal Representative
  • Sibling

Applications must be completed with full accuracy regarding the event and must be accompanied by the appropriate fee and identification of requestor. Please see below for full instructions and listings of proper identification and the application necessary to obtain certified copies.
